For Patients
If you’ve decided to visit us for your oral and maxillofacial needs, thank you! We’re very excited to schedule your first patient visit and get you familiar with our dental office. To make the process as smooth as possible, we’ve included all the important information you need. This includes expectations for your first visit, appointment scheduling, financing, and necessary patient forms. Feel free to give us a call if you have additional questions about your first visit!
Your First Visit
Prior to your first visit, we ask that you take the time to review our dental office policies and to call us at if you have any questions. On your first visit to North Metro Oral & Implant Surgery, you will be asked to fill out some information that will allow us to begin your treatment. When you arrive at our dental office, please be prepared to provide us with information regarding your medical/dental history. We will also ask you to fill out several forms that will get you acquainted with our dental office.
On your first visit to our dental office, John W. Carey, D.M.D. will perform a thorough examination of your mouth, teeth, and gums to assess your current dental condition. We may or may not take X-rays during this time. After a complete evaluation of your current dental health, Dr. Carey will then develop an individualized treatment plan based on your particular situation. Once your examination has been completed, Dr. Carey will take the time to clearly explain both your diagnosis and any dental procedures that are recommended to restore your dental health.
Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24 hours’ notice if you need to reschedule your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24-hours in advance, you are subject to being discharged from the dental office.
Patient Forms
You may preregister with our dental office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our dental office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Dental Insurance
Our dental office is committed to helping you maximize your medical or dental insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full. If you have any questions about insurance, our courteous staff is always available to answer them.
Dental Financing
We accept both cash and credit card payments to cover your treatments. If you do not have insurance, there are always additional financing options that can help you pay for treatment in monthly, more manageable chunks as well! In many cases, these payments come with little to zero interest attached, creating less of a burden on your oral health needs. During your visit, make sure to talk to our patient coordinators about the costs involved for your treatment!
